Children 5-12 - $35 each
T – SHIRTS:
XSmall - $10 each
Small–XL - $12 each
XXL and UP - $14 each
Please include names and ages of children with your payment.
Send dues ASAP or before June 18, 2016.

• Follow signs to tollway Interstate 294 North to Milwaukee.
• Proceed North and exit on Lake Cook Road.
• Proceed to Milwaukee Avenue exit and turn left at the light.
• The hotel entrance is at the second light on the left side.
